The Multires modifier does not like changes on the low-poly base mesh very much. It can mess up the high-poly levels and add spikes and other abnormalities. You can try to select and separate the new extruded mesh, remove the Multires modifier from the separated mesh, then join it again with the body object (that has the Multires modifier). Make the body object the active object (selected last) to keep the Multires modifier. If this doesn't work then it's time for plan B. Plan B: Shrinkwrap Make a copy of the original unchanged object, remove the Multires modifier from the copy, add the inside of the mouth to the copy, add the Multires modifier. Finally, add a Shrinkwrap modifier and shrinkw

Okay, I finally figured out how to do it. I hope the explanation below can help other users. To understand the process in more detail, I recommend the tutorial "Blender Sculpting Tutorial: Full Advanced Creature Creation Workflow" [this video] by CG Boost on YouTube. With any model selected, in Sculpt Mode with the Lasso Trim tool, I change Trim Mode = Join and Shape Orientation = View in the header. Enable Use Cursor for Depth if you want to control the length of the model in proportion to the radius of the brush. The smaller the radius, the thinner the object will be. Your retopo attempt is looking fine. Try to Shade Smooth (right-click in Object Mode ) and it should look much smoother. You have created a low-poly mesh (step 1c, see below). If you want to animate/pose the character, you will also need to retopo the body or use a base body mesh and add the head to it. Next, you can add a MultiRes modifier if you want and then start texturing. A common workflow the following: use a low-poly base mesh to form the shape of the character. This can be done in Sculpt Mode or Edit Mode . You have several options here: a) you can use an existing base mesh and shape it in Sculpt Mode b) OR box model it [LINK] from scratch ( Edit Mode ) c) OR sculpt it [this video] from
